Registration Process

Online Application

Register on the Swavlamban UDID portal and fill in personal and disability details.

Document Upload

Upload scanned copies of photo, signature, and proof of address/identity.

Medical Assessment

Visit the assigned District Hospital for assessment by the Medical Board (if new).

Card Dispatch

Once approved, the UDID card is generated and sent to your address by post.
